4. Ethanol and Neural Crest Cells

There is no consensus on a specific pattern of functional impairments due to prenatal alcohol exposure19 and only CDC guidelines label developmental delays as such,71 so criteria (and FASD diagnoses) vary somewhat across diagnostic systems. 3D imaging is already capable of identifying subtle facial features that distinguish children with heavy PAE from controls with 97% specificity for FAS and 90% for pFAS94. Because face and brain development are highly linked5, it is not surprising that the detection of subtle facial dysmorphology is predictive of neurocognitive impairment, even in children without cardinal facial features of FAS or pFAS. Animal models and tissue-culture studies identified numerous potential mechanisms of alcohol induced craniofacial and brain defects. Previous experiments showed higher sensitivity during gastrulation stages to environmental toxin-induced birth defects, including alcohol exposure 9. Experiments on zebrafish embryos treated with ethanol during gastrulation and somitogenesis showed changes in critical signaling molecule and transcription factor expression, including the notch ligand, deltaB, and the proneural gene, neurogenin1 (ngn1) (Figure 1A–J).

Study Population

Fetal alcohol spectrum disorder (FASD) can be suspected in newborns based on the maternal history of alcohol use during pregnancy and compelling physical attributes present at birth. The infrequency with which it is assessed during infancy makes FASD evaluation in newborns a formidable diagnostic challenge. Our results displayed a wide variation in total scores among the study participants, particularly in the FASD group, with the obtained total scores in this group ranging from 4 to 14.
